This article explores workplace accidents as a significant risk in industrial environments, emphasizing that such incidents are not mere misfortunes but often the result of unsafe systems. The discussion covers the causes of accidents from human, equipment, and environmental factors, all of which interact and require planned prevention strategies. Understanding these causes is essential to designing effective occupational safety measures. The study uses a library research method, drawing from books, scientific articles, and national and international regulations. This approach allows for a comprehensive analysis of concepts, causes, and practical prevention strategies, aiming to bridge theory and real-world application. The goal is to support improvements in Indonesia’s workplace safety systems. Emphasis is placed on Occupational Health and Safety (OHS) not only as a legal duty but as a reflection of respect for worker dignity. A safe workplace goes beyond formal compliance—it requires continuous education, cultural change, and collaboration. Ultimately, the article aims to evaluate applicable and effective accident prevention strategies in industry, highlighting the importance of fostering a safety culture. A secure work environment boosts national industrial competitiveness and protects lives. Every preventive action, however small, honors human life and represents a shared moral responsibility.
